Fuel system problems might have contributed to the crash of an ultralight on Kaua'i March 11.
A preliminary report, just released from the National Transportation Safety Board, points to possible structural defects.
Pilot Gerry Charlebois of Kapahi and passenger Mark McKenzie of Ontario, Canada were killed when their aircraft crashed into the side of a mountain near Polihale Beach.
Charlebois, with decades of flying experience, was conducting a flight lesson at the time.
The NTSB report includes statements from another instructor that the ultralight had recently had problems suctioning fuel out of the main vent line. \
Charlebois had been observed that morning trying to remedy the problem.
Investigators are looking at video from a camera recovered in the wreckage as they continue to search for a cause of the deadly crash.
