inmate has died at O- triple C. This is the FIFTH death at an Oahu prison in just the past two months. KITV4's Nana Ohkawa is in the newsroom with more on our top story-- Nana? Officials believe 56-year-old inmate Catherine Callahan committed suicide this morning. She was in jail from March 27th of this year-- awaiting sentencing for terroristic threatening, assault, and violation of a temporary restraining order. Callahan was found HANGING in a single cell unit, during a routine inspection by correctional officers. She was pronounced dead shortly later. Officials say she was under suicide watch before, but had been taken OFF before allegedly killing herself this morning. Callahan was in general population at O-triple-C-- where inspections are not as frequent as in a special holding cell or suicide watch units. Given the recent HIGH number of deaths in Oahu prisons, The Department of Public Safety says it's reviewing its policies and procedures. 730 VERY DISAPPOINTED THAT THIS OCCURRED BUT I THINK WE NEED TO UNDERSTAND THE COMPLEXITY OF THE PROBLEM THAT THEY COME TO JAIL WITH WE NEED TO ADDRESS AFTER THEY COME OUR FACILITY 750 The department also wants to give additional suicide training to correctional officers. It has brought up having more cameras, and positioning them INTO cell units to increase prisoner surveillance. The investigation of this case is still ongoing.
