Molokai's famous Kanemitsu's Bakery has been shut down following a second failed health inspection.

The state Department of Health first suspended the company's permit back in June after an anonymous complaint.

Inspectors found dough mix kept in large, dirty containers.  They also found bugs in cooking equipment and large areas exposed to the outside environment.

The bakery has been fined $90,000.  Health officials say the facility must pass a checklist of requirement before it is allowed to reopen.

The pictures used in the video above was from the first inspection and Kanemitsu's Bakery says the violations from that inspection were fixed.
