Final Report Blames Commander In Ehime Maru, USS Greeneville Collision

The captain of the USS Greeneville submarine was largely to blame for colliding into a Japanese fishing boat in 2001, according to a final report from the National Transportation Safety Board that was released Wednesday.

Nine people on board the Ehime Maru died.

The NTSB said among the reasons for the fatal collision was "Cmdr. Scott Waddle's decision to order an emergency surfacing maneuver."

The report also noted the crew's, in particular the commanding officer's, "failure to adequately manage the civilian passengers (on board) so that they did not impede operations."

The Greeneville was hosting civilian VIPs when it collided with the Ehime Maru outside Honolulu on Feb. 9, 2001.

The U.S. Navy Pacific Fleet commander, Adm. Thomas Fargo, found Waddle guilty of two offenses: dereliction of duty and negligent hazarding of a vessel in April 2001.
